It was a video, a videos can lie.  All they said it was running in realtime but it might not be actually gameplay.

I can't wait to watch this on the 3DS screen.
I'm not 100% sold on 3D, but this trailer definitely makes sure you see what they are doing.

I've never played any other Metal Gear games. Since this is a prequel, would I be alright going into this one first?

To understand the series story arc, you only really need to play Snake Eater, Peace Walker, Twin Snakes, Sons of Liberty, and Guns of the Patriots. In that order. Portable Ops messes with the timeline and ultimately makes no sense so I would ignore it (you won't miss much) while the original 2D games on MSX (Metal Gear and Metal Gear 2) are oddly inconsequential. Everything you need to know about those games is explained in Twin Snakes. On a side note, I would recommend tracking down Ghost Babel (called Metal Gear Solid in NA) on Gameboy Color. It's not canon, but it's a pretty awesome game. Shame Konami never made a GBA or DS follow-up.

Yup, Snake Eater, Peace Walker, Twin Snakes, MGS2, and Guns of the Patriots. What's unfortunate is that MGS3: Subsistence (the good version) changes up the camera scheme for the BETTER while Twin Snakes and MGS2 have broken cameras. So it's going to be an inconsistent journey, but it's very worthwhile. From a gameplay perspective, I think MGS4 is the best game (you have tons of options) but from a story perspective, MGS3 is the best. From a mind-blowing perspective, MGS2 is pretty awesome.

Twin Snakes is the best game focused on Solid Snake.
This would be my PSN Trophy Card, but I guess I can't post HTML in my Signature. I'm the pixel spaceship, and I have nine Gold trophies.

Oh my god, are we calling that "mind-blowing?"  I have a few different expletives I would use.  To be honest, I would steer everyone away from that game.  It's fun during the game, and the story is alright, up until the last bit after the main character (who I shall not name, to avoid spoilers) gets buck-naked.  And then it just gets so bat-**** crazy (story wise) that I my jaw was on the floor thinking what crazy idiot thought this up?  Oh, Hideki.

Personally, I don't think you need to have played any of the other games prior to enjoy Snake Eater, and especially considering that there's no personal radar in the game, it may even be better in terms of gameplay.  When I first started playing, the lack of personal radar really freaked me out.
