holding it sideways like a NES controller and having to press a button on the back seems like it'd be awkward.

Why can't you? Sawmill accident?

I'd say Mario R could be done perfectly with the NES config, if you want more directions just turn the controller a bit to line one of the dpad directions up with your desired direction. A jumps, B runs. What more does Mario really need?

Spin jump + Get off Yoshi.  But that could surely be mapped to the rear B button, or if you were to do it my way, have that B button be run.

Hm, yes, getting off Yoshi could be a problem but the spinjump won'T be necessary in a 3d Mario I think.
