Also, the "little girl with a weapon who kills tons of people" concept is so close to cliché in Japanese culture that I'm convinced there must be something sexual in the appeal (this would be about the umpteenth anime/game I've seen which touts the concept).

Maybe I'm expecting the worst from people, but when it's hard to find an anime/game which doesn't try for as many shameless upskirt/ass shots as possible (*COUGH*PerfectDark0*COUGH*), I think expecting the worst is just plain common sense.

Pure marketing reasons. A Jack Rambo Hugging Pillow wouldn't sell much. As opposed to merchandise like this.
